I will set up my fax-on-demand machine with a sample affidavit for those
who want to know how to format it - it is over 20 pages long - but very
thorough and yours needs to be so, too. My fax number is 719-749-2972 -
but I'll need a little time to set it up.

2 - documentation to support your claims against social services. This
includes relevant court records, transcripts, case reports, tape
recordings (have these transcribed), court orders, etc.

Mail these to my publishing house - address below - include four
CERTIFIED copies of your affidavits, documentation and release forms:

department, medical facility, company, or individual may have relating

requested by Suzanne Shell or the Justice Department. I release the
agency, department, medical facility, company, or individual from any
and all liability for supplying such information.
Suzanne Shell and/or the Justice Department is authorized to publicize,
disseminate or use this information freely and shall be not be held
liable for taking such action by me or my assigns. This authorization is
not transferable or assignable to any other agency or entity.
This authorization remains in effect until rescinded in writing.
